What we need from you

In order to see a podiatrist or physiotherapist, you must get a DVA D904 referral from your GP, made out to Adelaide physio and Podiatry Clinic.

This referral will last for 12 months or a maximum of 12 visits, whichever comes first.

For an initial consultation allow 30-40 minutes.

During your appointment

Your initial consultation will include an assessment where your podiatrist or physiotherapist will determine your course of treatment, beginning immediately.

What are the costs involved?

This will depend on the service you require and what card you hold. There are usually no out-of-pocket costs for DVA gold card clients, but please discuss this with your podiatrist or physiotherapist at your initial appointment.
